Speech discrimination impairments as a marker of disease severity in multiple sclerosis

چکیده
BackgroundMultiple Sclerosis (MS) pathology is likely to disrupt central auditory pathways, thereby affecting an individual's ability discriminate speech from noise. Despite the importance of discrimination in daily communication, it's characterization context MS remains limited. This cross-sectional study evaluated under "real world" conditions where sentences were presented ecologically valid multi-talker or broadband noise at several signal-to-noise ratios (SNRs).MethodsPre-recorded Bamford-Kowal-Bench five (SNR) one two background noises: speech-weighted and eight-talker babble. All stimuli via headphones control (n = 38) listeners with mild 20), moderate 16) advanced 10) disability. Disability was quantified by Kurtzke Expanded Status Scale (EDSS) scored a neurologist. participants passed routine audiometric examination.ResultsDespite normal hearing, psychometric curves which model relationship between ratio sentence accuracy babble did not change slope (sentences/dB) but shifted higher SNRs (dB) compared controls. The magnitude shift curve systematically increased greater Furthermore, mixed-effects models identified EDSS score as most significant predictor (odds 0.81; p < 0.001). Neither age, sex, disease phenotype duration significantly associated performance Only disability self-reported audio-attentional difficulty questionnaire designed reflect processing behaviours life.ConclusionSpeech worsened disability, independent education, phenotype. These results identify novel deficits highlight that tasks may provide viable non-invasive sensitive means for monitoring MS.

decreased urinary level of melatonin as a marker of disease severity in patients with multiple sclerosis.
melatonin has both pro-inflammatory and anti-inflammatory properties depending on the stage of inflammation. despite its therapeutic effect in alleviation of some symptoms of multiple sclerosis; the precise role of melatonin in ms pathogenesis remains a topic of debate. the aim of this study was to measure the urine level of one of melatonin products which is an index of serum melatonin level, ...
متن کاملCorrelation between Acoustic Parameters and Disease Severity and Duration in Patients with Multiple Sclerosis
Background: Since in multiple sclerosis (MS), changes in speech and voice quality often precede other signs and symptoms; early diagnosis of these changes is necessary. In this study, an acoustic examination of phonation subsystem was performed. Due to the progressive nature of multiple sclerosis, the aim of this study was to examine the correlation between acoustic parameters ...
متن کاملMotor Imagery as a Function of Disease Severity in Multiple Sclerosis: An fMRI Study
Motor imagery (MI) is defined as mental execution without any actual movement. While healthy adults usually show temporal equivalence, i.e., isochrony, between the mental simulation of an action and its actual performance, neurological disorders are associated with anisochrony. Unlike in patients with stroke and Parkinson disease, only a few studies have investigated differences of MI ability i...
متن کاملA Marker of Disease Severity?
Over the last 2 decades, major advances have been made in our identification and understanding of the genetic basis of cardiovascular disease. More than 40 cardiovascular disorders have now been identified to be directly caused by single-gene defects. These disorders span all aspects of cardiovascular disease and affect all parts of the heart structure. They include the inherited cardiomyopathi...
